Application Checklist

 

New Student? Here is what you need:

A. Please review the following on our web site:

B. Complete and/or compile the Application Packet Information listed below:

1. Application (*Returning Students please see below.)
2. Private/Parochial School Form
3. EdChoice Form & Information Sheet (if applicable)
4. Medical History & Shot Records
5. Request for Student Records
6. Expulsion Information Sheet
7. Birth Certificate
8. Report Card (most recent)
9. Proof of Residency
10. Transportation Form
11. After-School Program Enrollment Form (if applicable)

C. Pay the non-refundable Application Fee of $50.

D. Make an appointment for the following:

• Testing
• Interview (Note that students in grades 6-8 should be present in the interview.)

School policy handbooks are available in the office. Please print and complete the forms above. Return them to the office by mail or in person.

At your request, we will send you the forms. All of the above items must be completed and turned in to the office before your child may be enrolled at Central Baptist Academy.

*Returning Students:

Returning students do not pay the $50 new-student application fee. That fee covers the initial application, interviewing and testing process for new students only. However, returning students must fill out an registration form each year. Please get your registration form in early to be sure that your child does not get closed out of a filled class. Questions?
